Treatment Model Policy

Thank you for choosing to be a treatment model at Avevya Institute. Our treatment model program allows qualified healthcare providers to gain hands-on clinical experience under the direct supervision of experienced instructors and qualified providers while offering models high-quality aesthetic treatments at significantly reduced pricing.

Because treatment models are an essential part of our educational programs, we ask that all appointments be taken seriously and that the following policies be observed.

Good Faith Exam

In accordance with Texas regulations, all treatment models are required to complete an annual Good Faith Examination (GFE) prior to treatment.

• Good Faith Exam Fee: $25 (required annually)
• The Good Faith Exam fee is separate from the appointment deposit.
• Completion of a Good Faith Exam does not guarantee treatment eligibility.

Treatment recommendations are based on medical history, provider assessment, and patient safety.

Appointment Deposit

A $50 non-refundable appointment deposit is required to reserve all treatment model appointments.

The deposit will be applied toward the cost of your treatment at the time of your appointment.

Appointment Rescheduling

Treatment model appointments may be rescheduled one (1) time provided we receive at least 72 hours' advance notice.

Your original deposit may be transferred to the new appointment date.

Rescheduled appointments are subject to availability.

Late Rescheduling

Requests received with less than 72 hours' notice require a new appointment deposit to schedule another appointment.

The original deposit will be forfeited.

First-Time Treatment Models

Because our educational programs rely on committed treatment models, first-time treatment models who cancel their initial appointment for any reason will forfeit their deposit and will not be eligible to schedule future treatment model appointments with Avevya Institute.

Cancellations & No-Shows

Appointments canceled with less than 24 hours' notice, same-day cancellations, or failure to appear for a scheduled appointment (no-show) will result in:

• Forfeiture of the appointment deposit.
• Ineligibility to participate in future treatment model appointments at Avevya Institute.

These policies ensure fairness to our students, instructors, and other models waiting for appointment availability.

Guests

To maintain a safe and professional learning environment, guests, children, and additional visitors are not permitted during treatment model appointments unless prior approval has been granted by Avevya Institute.

Treatment Model Information

Treatment model appointments are conducted as part of a live educational training program.

By participating, you acknowledge and understand that:

• Students perform procedures under the direct supervision of experienced instructors and qualified providers.
• Appointment times are approximate and may take longer than a traditional clinical appointment due to the educational nature of the training.
• Treatment plans may be modified based on clinical judgment, patient safety, product availability, or educational objectives.
• Certain procedures may be postponed, modified, or declined if determined to be clinically inappropriate or if safety concerns arise.

Patient safety will always take priority over completion of any procedure.

Treatment Eligibility

Not every individual is an appropriate candidate for every treatment.

Avevya Institute reserves the right to modify, postpone, or decline treatment based on:

• Medical history
• Current medications
• Contraindications
• Safety concerns
• Provider assessment
• Clinical judgment

If an alternative treatment is recommended, it will be discussed prior to treatment.

Refund Policy

All treatment model fees, appointment deposits, and services are non-refundable, non-transferable, and non-exchangeable.

No refunds will be issued for completed services or missed appointments.
